PHLEGM v8 Checkpoint 37 — SuperPET origin-aware core dump tracing

Source baseline: PHLEGM v8 CoreDump Checkpoint 36.
Updated source: src/lib/coredump.bi, src/lib/vm-exec.bi, src/lib/monitor.bi.
Trace: first 256 and latest 4096 screen/bank-latch writes; captures instruction-start PC,
compiled opcode key/mnemonic, optional effective-address and last-read hints, raw and
aliased bus addresses, bank/latch selection, physical before/after values, and
actual bus dispatcher route including rejected writes.
NMI/IRQ context is separately identified; last-read hints are NOT guaranteed
copy-source addresses. Original .session.phlegm and .phlegm micro-code are unchanged.

Validation: static checks, 12 BusWrite exit paths, console/GUI command availability,
CRLF/LF line endings, ZIP member CRC, and no shipped Python — PASS.
Native FreeBASIC compiler/link/runtime testing: NOT RUN (fbc unavailable here).
No assertion that display corruption is fixed until the first-write dump is captured.
